  Hoop Masters Basketball Academy Sign UpHMBA REGISTRATION INSTRUCTIONS 2007-2008 Please Note: If you are not sure of the schedule, refer to the Levels Section of the HMBA Overview. Once you’ve identified your child’s level, go to HMBA’s Calendar to find the corresponding nights and Locations for that level of practice. So that we are aware of your attendance, please either call 310.857.6400 or email info@hoopmasters.org. We can then staff the practice appropriately. Complete the forms below and email or bring them with you to practice. All of the completed forms and payment is expected at the first session you attend. Checking the Schedule Link on the website will provide any last minutes changes at the gym you will be attending. The HMBA forms listed below are due the first week your child participates in our program. It is important to complete all the required forms to ensure your child’s participation.

If you are interested in participating in Hoop Masters but need financial assistance, please download, read, and complete Scholarship Request Package. ALL SCHOLARSHIPS WILL BE APPROVED BY THE HOOPMASTERS SCHOLARSHIP COMMITTEE ON AN ANNUAL BASIS. IF YOU WERE ON A SCHOLARSHIP LAST YEAR - YOU MUST REAPPLY. CLICK HERE. After your child is enrolled, print and complete the appropriate form. Return it with your payment, so that will receive the practice jersey and uniform appropriate to his/her level. |
